- Date of birth
- Nationality
- Destination in Italy (Shooting range and denomination of the championship)
- Reason of the trip in Italy
- Exact dates of arrival and departure in Italy
- Description of the gun: type (shotgun), model, Brand, caliber, serial #, other details (extra barrels)
- Email adress and/or fax
FITAV will send you a copy of the permit by fax or by email. You will have to show that copy of permit to the customs upon your arrival in Italy.
